Complete the online Derived Exam Rating Application and Consent form for students. In this form, you need to:
- provide your TASC ID [see your Notice of Enrolment (NoE), also called a ‘pink slip’, which has your external assessment timetable]
- detail the exam/s you are applying for derived exam ratings for
- make a declaration that your application is correct and allowing your medical information to be provided to TASC.
After completing the online form, make an appointment for a TASC Medical Certificate to be completed by a medical practitioner (a registered GP, paediatrician or a hospital doctor only).

Be aware of the application time period: You can only apply for derived exam ratings in the specified time period. For illness, this is no more than seven (7) days before OR two (2) business days after the written exam, practical/performance or presentation. Your medical appointment and submission of the online Derived Exam Rating Application and Consent Form must BOTH be within the application time period to be accepted.
You cannot apply for derived exam ratings for a written exam until 3 November at the earliest.
